1: Add a key to the array in $sourcedir/ManagePermissions.php
2: Add some language strings to the array $txt in ManagePermissions.engilsh.php
3: Drunk, may lose my lunch ;)
4: Add isAllowedTo() function calls and suchlike to your action

Really I've covered two separate things here., and in a very informal way. Rhis is why I've put it on the talk page though and not on a topic page.

PS I had to add XHTML line breaks by hand because the wiki didn't honour my newlines (\n).. Don't know if this is intentional. Unbuntu Hardy, Firefox 3.0b3pre, possbily a stupid beta issue.
